Submit a Sample
Each sample submitted to Infinity Laboratories must be accompanied by a Sample Submission form and the appropriate labels, documentation, and instructions for testing.
Download, complete, and submit the form corresponding to the site conducting your tests. If your testing will be performed at our St. Louis, MO Midtown lab, please access the Infinity Midtown LabVantage LIMS portal.
Samples are received Monday through Friday (excluding company-recognized holidays) during each facility’s operating hours.

You can pay your Infinity invoice via an ACH or credit card payment. If you are not currently set up to make ACH payment to Infinity, please email ar@infinitylaboratories.com.
